Step 2: Please fill out Part A with black ink only

Step 3: Please email carlosb@fultonschools.org or return to student services

Step 4: Completed forms will be approved Friday after 11:30 pm

If a student/parent requests a DDS form (for proof of enrollment at the license office) please advise the student/parent to come to school to complete portion A then they can pick up the completed form 72 hours later in the front office.
